Preparing for Your Appointment

You will need to call the pre-registration office to provide contact and payment information if you are not currently a patient at the UCLA Medical Center or a UCLA doctor has not seen you in more than a year. Otherwise, your consultation may be delayed as you will have to register on your arrival before being seen by the doctor. We recommend that you pre-register at least 2 days before your appointment. |

What you should bring
Please bring the following to your initial visit:
- Eyeglasses
Bring the eyeglasses you wear most often and/or your contact lens prescription with you, even if they no longer help your vision. Old eyeglasses and contact lens prescriptions are also helpful to bring as they will provide important information about the past condition of your eyes.
- New Patient Questionnaire
Please bring the completed new patient questionnaire with you to your visit.
You may have received these forms during one of our seminars or through the
mail. You can also download the forms from this website. If you do not have
the completed forms, you will be asked to complete them upon arrival.

Special preparation for contact lens users
Remove your soft contact lenses at least 3 days before your visit and keep them out until the appointment is over. Rigid gas permeable (RGP) and other hard contact lenses should be kept out for 3 weeks.
